It's a profile picture, theres an option to upload one in your UserCP somewhere, I don't remember exactly where off hand.

im assuming you mean like sharp has match corrdinator in his

 

thats a custom user title you can get one here

thats an avatar and can be uploaded in your user control pannel

 

under the "edit avatar" section
